Here's a primer on why it's worth learning how to use INDEX and MATCH rather than just sticking with VLOOKUP. <h2> How to Use VLOOKUP</h2> First, a quick reminder of how VLOOKUP works. A blood test that recognizes such a biomarker for in the brain could “revolutionize clinical trials today and in the future,” says Maria Carrillo, chief science officer of the Alzheimer's Association. “And think of the implications for clinical practice once we have a therapeutic.&quot; In that event, as she describes it, patients could one day get a quick blood test during their to see if they have elevated biomarkers for Alzheimer's. Those who do would then be referred to a specialist to begin a course of treatment as soon as possible. In the new study, researchers at the National Center for Geriatrics and Gerontology in Japan developed a prototype for a blood test that “reliably” picked up the accumulation of amyloid precursor protein (APP) with 93 percent specificity in the blood of 201 patients with mild cognitive impairment, Alzheimer's disease, or non-Alzheimer's . It also picked up the protein in some subjects who were cognitively normal. (While amyloid plaque is considered a reliable biomarker of Alzheimer's, some of those with the plaque never go on to develop signs of the disease.) Boost Your Brain Health With Last year the same group of researchers found that blood biomarkers of this kind of amyloid protein were as reliable as the current gold standard of diagnosing Alzheimer's with positron-emission tomography (PET) or measuring the protein through cerebrospinal fluid. A blood test is much less invasive and more cost effective than a PET scan. The new blood test developed in Japan may also be more efficient at picking up preclinical stages of Alzheimer's disease, according to the study. This would enable earlier intervention. Currently, changes in amyloid protein can exist for as long as two decades before the point at which a patient would be clinically diagnosed with Alzheimer's disease. &quot;Clinical trials haven't succeeded up until now, likely because we need to find a novel therapeutic and be able to catch people early in the disease before it is established and becomes a runaway train,” says Barry Greenberg, director of the Alzheimer's Disease Translational Center at Johns Hopkins School of Medicine. Some of those hearing news of the blood test remain cautious, citing the need for the study to be replicated in a larger, more diverse population, or noting that a blood test that detects other Alzheimer's biomarkers, such as viruses or a protein called tau, might turn out to be better biomarkers to target in a blood test going forward. We Americans adore our . We love our smart cars. Attachable bidet (pronounced bih-DAY) seats — hands-free, high-tech marvels that you can install on your existing throne to gently wash and dry your private parts — are growing in popularity, particularly with boomers upgrading the bathrooms in their , or splurging on their new, downsized digs.<br /> <br /> The seats also can be a lifesaver for older people who need help using the toilet, says Mary Tinetti, M.D., head of geriatrics at the Yale University School of Medicine.<br /> Courtesy TOTO Toto remote panel allows users to control the water spray as well as the dryer function. Celebrity fans Toto, the Japanese company that's the world's largest manufacturer of bidet-style toilets and add-on seats, has seen U.S. sales of its Washlet seats steadily increase since they were introduced in the United States in the 1990s. (In Japan, the Washlet is so ubiquitous that its name has become virtually synonymous with the word toilet.) Although David M. Krakoff, head of North American sales for Toto, won't reveal exact figures, he says sales have grown annually by nearly 20 percent for the past decade. Sales spiked 50 percent in 2010 with the introduction of new models and showrooms. It hasn't hurt that the Washlet has its vociferous fans among celebrities, including Whoopi Goldberg, who installed one in each of the six bathrooms in her New Jersey mansion and yakked about it on The View, as well as Bryant Gumbel, Howie Mandel and Brad Pitt, according to Toto. The Kohler Company, a Wisconsin-based manufacturer, introduced what it calls &quot;toilet seats with bidet functionality&quot; in 2006. Although the company also won't reveal sales figures, spokesman Shane Allis confirms an increase in the &quot;overall bidet market.&quot; The devices, which run from $650 to $1,800, have remote-controlled retractable wands that spray well-aimed, warm, aerated water to clean you front and back. A dryer emits warm air, which reduces or eliminates the need for toilet paper, and an air purifier absorbs any unpleasant odors. The lid even closes quietly when you're done — a function one wag called &quot;the marriage-saver.&quot; In addition, nearly every option can be customized, from the temperature of the seat, water and air, to the pulsating of the spray and the speed of the dryer. Depending on the model, you can also have a lighted bowl to guide you in the dark. Plus, the seats can save you money, both on toilet paper and water, because they often require less water to flush.<br /> <br /> Next: Hygiene help Health experts say bidet seats are better for maintaining good hygiene, particularly for those who have or physical mobility issues. They're especially helpful for seniors who may have some physical limitations that make &quot;wiping difficult or painful,&quot; says Yale University's Tinetti. The better cleaning you get from bidet seats may also help reduce the bacteria that cause urinary infections among older women, Tinetti adds. Although Kohler has a variety of products it calls &quot; solutions,&quot; Allis says the bidet seats &quot;are being used across all age groups.&quot; &quot;Some see them as a way to use less toilet paper and be environmentally friendly, while others see it as an overall hygiene improvement and a way to stay in their home longer,&quot; he says. Cheryl Smarr with the Ferguson Bath, Kitchen and Lighting showroom in Austin, Texas, says she's selling more bidet seats &quot;with all the amenities&quot; to couples in the 50-plus age range, and not just because the wives want them. &quot;We have as many of the husbands requesting the bidet seats as we do the wives,&quot; she says. Debbie Nassetta, an interior designer in Laguna Niguel, Calif., says she's been surprised by men's enthusiasm for the smart-function toilets. &quot;Men are definitely more interested. Searching for BioWare, somewhere in Anthem&#39;s Fort&nbsp;Tarsis Grabbit and don&#39;t let go. Feature by Tom Phillips Deputy Editor Updated on 26 Feb 2019 106 comments BioWare has been asked - a lot - why it is the studio to make Anthem, a game far removed from its traditional single-player role-playing game fare. BioWare's responses have been - 'well, we've done multiplayer before', 'we've experimented in other genres before', and also 'would you please let us make something new for a change?' And fair enough. But from day one, many BioWare fans have been turned off by the idea of a game not specifically built for them. (And I'm sorry BioWare, as much as you protest 1996's Shattered Steel had multiplayer too, single-player RPGs are the million-selling corner of the market you have pretty successfully endeared yourself to.) Yet a huge chunk of Anthem has been built specifically for BioWare fans - a chunk which is rigidly single-player, follows close to the studio's recent formula, and where BioWare's loremasters have been crafting characters and story which feel very familiar to the ones fans have previously fallen in love with. This portion of the game, Fort Tarsis, is a classic BioWare hub. And, for whatever reason, BioWare has barely mentioned any of it. Anthem is being sold on its team-based shooting, its loot game and its shiny mech suits, but the part of Anthem I've found myself enjoying is the stuff you do when all of this goes away. When I step out of my mech suit, onto Fort Tarsis' cracked cobbles and through its Moroccan arches, across its bleached plazas, down stairways flanked by uneven stones, into its dark undercrofts. Here you'll find Anthem's quest givers and random NPCs, lore tabs and collectibles. Here you'll find a warmth, humour and depth not found anywhere outside its walls. Owen, left, is the game's expressive standout character. Anthem's population, weird and colourful, are lounging and leaning, waiting for you to pass the time. Amongst your regular contacts are passers-by, there to share a story if you'll listen, of their place in the world and how they're surviving in it. BioWare has never spoonfed its lore but presented a banquet of it, the strength of your appetite the only blocker to how many of its sci-fi sounding words you care to understand. Cypher. Corvus. Urgoth. And where Mass Effect has space hamsters and Dragon Age has nugs, Anthem has grabbits. Any time you return from a mission you'll find conversations highlighted on your map ready to begin. Quest givers like Haluk and Tassyn slowly open up to you over time, warming in a way which reminds me of Dragon Age's Cassandra. Side characters like Jani and Kismet are the start of Anthem's wider story outside the game in spin-off media. Others fill in backstory on the world's history, geography, culture - or just remind you there are some unusual people around. Neeson Giles is a creep, but I can't help talk to him every time I hear him chattering away nearby. He's a gossip, and typically if you've just gotten closer to one of your main allies, he'll have heard something poisonous about them he can't wait to share instead. His job is to do the fort's laundry, and he's bitter about it. Pirndel Blatch, meanwhile, is the fort's custodian but spends his time moaning about health and safety. He's the Facebook friend who would love some more support with their petitions. Grabbits. A section of Anthem's children's book Tessi the Tesilar "It's not that, Grabbit. It's just that I feel on edge, and when I fall off it, I don't want to land on you." "Don't be silly, Tessi," Grabbit scolded. "Everyone knows that when you're on edge, friends are the best parachute." Wander the fort's passageways and you'll find scrolls to read or radios broadcasting the game's versions of children's stories and pulpy romance. Every mission, every character, every gun has a log page, kept constantly updated as more about each is uncovered. Navigate Anthem's labyrinthine UI further and you'll belatedly uncover you have a fictional email inbox (finding this has never once been prompted to me) with its own spam section. It's a pretty funny read. To be clear, there are no characters here with the diverse backgrounds of Mass Effect's many alien species, or Dragon Age's fantastical races. And your choices with them are limited - you colour their stories through smaller binary dialogue choices, rather than make big life or death decisions. But in Anthem's weird world, some of these more intimate, more mundane choices stand out - encouraging people to not give up hope, to maybe try sobering up, and that yes you will sign that petition, I suppose. It's hard to know how much BioWare fans will hear about this part of the game, and how it has been built for them to explore. Certainly, there's been plenty to talk about in early impressions and reviews about the main Anthem gameplay loop and how successful BioWare has been at nailing that. But I'd encourage BioWare fans to try Anthem someday for its Fort Tarsis stuff. There's half a BioWare game here they'd like, if they know to look. It's probably a question that every conscious generation has grappled with going as far back as, well, who knows? But it seems like a more pertinent, gripping question now than it ever has been. Maybe that's just my human brain and its inability to think broadly, but there's something about the digital era we're living in that makes this central question feel more depressing than I imagine it would if I were a farmer in 1800s Cyprus, frolicking in the sun and bathing on the shores. "I looked at the internet for too long today and started feeling depressed." My brain constantly flips between headlines that read "World War III imminent", to "New Mouthful mode in Kirby", all while my face reacts like nothing has happened. Scrolling through words that are at once world-ending, then hilarious renders all of it a little null. Finding true meaning, at this point in time, has never felt as essential as it does now when we're faced with the meaningless black hole of a blue screen in one hand and the collapse of the Earth in the hands of, I don't know, God? Watch on YouTube Nier Automata launch trailer. In times of existential dread we turn to art, aka food for the soul, for answers to the question, where do we find meaning in a meaningless world? And what I found was that Platinum Games' Nier Automata and Sally Rooney's Beautiful World, Where Are You both have an answer to that question. The synapses in my brain were able to connect the two during a 3am overthink and what I found was, weirdly enough, that they have exactly the same answer. Rooney is a famous Irish novelist and her third novel, Beautiful World, Where Are You, primarily follows Alice and Eileen as they begin new relationships and deal with midlife crises. So, what could that possibly have to do with Nier Automata, Square Enix's fan-favourite action-RPG? Everything, is the answer. "It always ends like this." Director Yoko Taro has long dealt with big, philosophical ideas clothed in whacky JRPG goofiness, and Automata sprints with that idea like the world is ending, because it is ending. Nier Automata sees 'androids' and 'machines' fighting for dominion over Earth on behalf of exiled humans on the moon and aliens that preside over the planet. After countless battles, upgrades, side-quests and other JRPG vignettes, it's revealed that both the humans and aliens have long been extinct. After a couple more hours you find out the head of the androids already knew this, they just didn't want everyone else to lose meaning in their existence. Sally Rooney at Waterstones, Piccadilly.. It's a crushing revelation in the context of the game. The high-octane melodrama of the JRPG dissipates. All that's left is a kind of emptiness. The knowledge that life, at least in this game, is an endless cycle of war to no end. Once faced with this realisation, the machines in Automata begin to mythologise and reenact the ancient culture of humans - not realising they've already done so through the act of violence. One baffling sequence has the barrel shaped machines awkwardly emulating sex. Awkward might be an understatement. The most interesting case of this comes as the machines don religious outfits and begin to pray to a central machine. Once its head falls off they become unhinged, chanting "Become as Gods." Either they're manically resisting the meaninglessness of their existence or this death is seen as a sort of martyrdom, ascending to Become as God. "Please God show me what you want." Rooney similarly depicts a feeling of meaninglessness. The characters in Beautiful World, Where Are You, search for purpose in all kinds of post-postmodern ways: in literature, social media, drugs, sex, religion. All they're left with, in every page, are more questions, struggling to understand each other or themselves. The usually omniscient narrator doesn't even have answers, itself constantly interpreting facial expressions or gestures. Modern structures of economics and digitisation make Alice and Eileen dissociate themselves from their own sensory experiences. What makes this worse is the knowledge that the impending doom of humanity has already happened before in the Late Bronze Age collapse. What they're living through isn't unique. Even the way they communicate - through email exchanges - echoes the epistolary form that many of the earliest novels used. If the androids and machines are repeating the cycles of humanity, what are we doing? The surface of both texts is wildly different but they're both depicting worlds deprived of structure for the people that inhabit them. The only difference is one of them exists in a world full of crazy anime battles and one of them exists in the real world, our world. "Do you think games are silly little things?" So&hellip; Taro makes us delete our saves and Rooney goes on a multi-page rant about the pointlessness of the contemporary novel, while we're reading her contemporary novel - both forcing us to acknowledge this meaninglessness. Great. Life is meaningless and there's still three endings to go in this game and 100 pages left of this book. Except, right at the end of both, Taro and Rooney seem to shift their stance. In the face of personal and universal ruin, in the face of existences that have been cosmically stripped of purpose, these characters hold onto each other. The only beauty they can find in the world is in the people they love. Nier Automata pulls off an ending that only a video game as boisterous and confident as this could. Our central characters kill each other fighting for ideas that the game has long-established as pointless. Credits roll. The end. Then, weirdly, the control is put back in players' hands as we attempt to literally kill the credits of Taro and the development team, bullet-hell style. This is a near impossible feat as the credits fire relentlessly. After dying, repeatedly, something magical happens. The ghosts of other players appear, words of encouragement typed in the background, from all over the world. "I never quite realised&hellip; how beautiful this world is." Beautiful World ends, somewhat controversially, in the same way many 18th Century novels do, with Eileen happily married and pregnant. After spending approximately 300 pages resisting these traditional structures, the only way that she finds happiness is by investing in her own world. Eileen's U-turn worked for me but has left countless readers unsatisfied, if Goodreads reviews (the Metacritic user review of the book world) are anything to go by. Depending on the interpretation, Eileen's transition to happiness is either a call to cling to each other in times of crisis, or it's supposed to make readers feel as unfulfilled as Jane Eyre and Little Women do, this time in a meta-fictive way. Again, reminding readers that there is no long-term comfort to be found with or without structure. If we read the book with the former interpretation in mind, it's clear that it hasn't worked for many readers. It seems even one of today's most famous novelists is unable to make the corny message of 'live for love' work. Maybe Alice was right, the contemporary novel is no place to talk about real people while the world burns. But maybe video games are. Automata's ending is universally adored, partly because it does what no other medium can do. It doesn't just call for people to support and hold and love one another, it makes us. It puts us side by side, killing the makers, protecting each other, encouraging each other and sharing something, even if it is just a game. There's at least some beauty in that. "As long as you both live the world will be beautiful to me." This compared with 50% and 30% in the 2014 and 2007 surveys, respectively. Irrigation bottle infusion and intravitreal injection was each used by only 5% of respondents. For IC antibiotics, vancomycin was used by 6% in the United States (52% in 2014), compared with 83% for moxifloxacin (31% in 2014). Equal numbers used compounded moxifloxacin or the Vigamox bottle as the source. There was a decrease in respondents using preoperative (73% from 85%) and postoperative (86% from 97%) topical antibiotic prophylaxis; the latter was not used by 24% of surgeons injecting IC antibiotics. Reasons cited by those not using IC antibiotics include mixing/compounding risk (66%) and being unconvinced of the need (48%). However, 80% believe having a commercially approved IC antibiotic is important; if reasonably priced, this would increase adoption of IC prophylaxis to 93%. We report the phase III IMpassion050 primary analysis of neoadjuvant atezolizumab, PH, and chemotherapy in these patients. METHODS: Patients with a primary tumor of > 2 cm and histologically confirmed, positive lymph node status (T2-4, N1-3, M0) were randomly assigned 1:1 to atezolizumab/placebo with dose-dense doxorubicin/cyclophosphamide, followed by paclitaxel, and PH. After surgery, patients were to continue atezolizumab/placebo and PH (total: 1 year of HER2-targeted therapy); those with residual disease could switch to ado-trastuzumab emtansine with atezolizumab/placebo. Coprimary efficacy end points were pathologic complete response (pCR; ypT0/is ypN0) rates in intention-to-treat (ITT) and programmed cell death-ligand 1 (PD-L1)-positive populations. RESULTS: At clinical cutoff (February 5, 2021), pCR rates in the placebo and atezolizumab groups in the ITT populations were 62.7% (n = 143/228) and 62.4% (n = 141/226), respectively (difference -0.33%; 95% CI, -9.2 to 8.6; P = .9551). The pCR rates in the placebo and atezolizumab groups in patients with PD-L1-positive tumors were 72.5% (n = 79/109) and 64.2% (n = 70/109), respectively (difference -8.26%; 95% CI, -20.6 to 4.0; P = .1846). Grade 3-4 and serious adverse events were more frequent in the atezolizumab versus placebo group. We compared the mean score for the same individual instrumental activities of daily living (IADL), frequency of going out, exercise, and social interaction at two-time points. We also examined the influence of living arrangement (living alone or not) on the frequency of exercise and social interaction. Results: The frequency of going out decreased during the pandemic (in 2020); however, there was no significant change in IADL. The frequency of exercise and social interaction increased irrespective of the living arrangement. The frequency of exercise increased more in those living alone. Conclusions: Although older adults refrained from going out, they compensated for the risks of inactivity in daily life by increasing or maintaining their frequency of exercise and social interactions. Knowledge of tracheostomy practices and outcomes of ECMO-supported children who undergo tracheostomy on ECMO may inform decision-making. DESIGN: Retrospective cohort study. SETTING: ECMO centers contributing to the Extracorporeal Life Support Organization registry. PATIENTS: Children from birth to 18 years who received ECMO support for greater than or equal to 7 days for respiratory failure from January 1, 2015, to December 31, 2019. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Three thousand six hundred eighty-five children received at least 7 days of ECMO support for respiratory failure. The median duration of ECMO support was 13.0 days (interquartile range [IQR], 9.3-19.9 d), and inhospital mortality was 38.7% (1,426/3,685). A tracheostomy was placed during ECMO support in 94/3,685 (2.6%). Of those who received a tracheostomy on ECMO, the procedure was performed at a median 13.2 days (IQR, 6.3-25.9 d) after initiation of ECMO. Surgical site bleeding was documented in 26% of children who received a tracheostomy (12% after tracheostomy placement). Among children who received a tracheostomy, the median duration of ECMO support was 24.2 days (IQR, 13.0-58.7 d); inhospital mortality was 30/94 (32%). Those that received a tracheostomy before 14 days on ECMO were older (median age, 15.8 yr [IQR, 4.7-15.5] vs 11.7 yr [IQR, 11.5-17.3 yr]; p =0.002) and more likely to have been supported on venovenous-ECMO (84% vs 52%; p = 0.001). Twenty-two percent (11/50) of those who received a tracheostomy before 14 days died in the hospital, compared with 19/44 (43%) of those who received a tracheostomy at 14 days or later (p = 0.03). CONCLUSIONS: Tracheostomies during ECMO were uncommon in children. One in four patients who received a tracheostomy on ECMO had surgical site bleeding. Children who had tracheostomies placed after 14 days were younger and had worse outcomes, potentially representing tracheostomy as a "secondary" strategy for prolonged ECMO support.
